Learning From Disasters
In response to inquiry recommendations and extensive consultation with the CEO, Chiefs, CMSHAC, MSHAC, BOE and mining industry training and safety experts, the Learning from Disasters course created by Simtars, on behalf of Resources Safety and Health Queensland aims to be one part of a broader solution.

The Learning from Disasters course has been created by Simtars, on behalf of Resources Safety and Health Queensland for statutory ticket holders to meet the requirements for continuing professional development.
The course blended delivery model combines three (3) eLearning content blocks and a one (1) day face-to-face workshop. Key topics have been developed in response to coronial and other independent investigations and reviews, legislative updates and extensive consultation from industry and health and safety professionals in the resource sector.
Learners will build their capacity to:
- Identify the pattern causes which repeatedly recur in mining incidents.
- Investigate the term ‘drift into failure’ and build awareness of organisational normalisation of signals of danger, otherwise known as disaster incubation.
- Identify how mining legislation relates to their own obligations and how to discharge them in a practical sense.
- Identify the consequences of poor behaviours or inadequate resourcing.
- Understand the safety challenges of modern technologies and subsequently identify why a theory of HRO is needed.
The face-to-face 1-day workshop focus: To establish tools and actions that Learners can implement in their own workplace at all levels, to strengthen safety frameworks and culture.
Through personal and workplace reflection, activities and collaborative discussion, participants will learn about behavioural and thinking biases that can affect your decision-making and attitudes, unpack the major ideas that shape health and safety practices and consequently improve their investigation and learning techniques. Key concepts of HRO theory introduced in the eLearning will be further unpacked to identify what they mean practically in terms of site behaviour – collective mindfulness, redundancy and flexibility.
Learners must finish: – All online course modules with Simtars and – Attend in-person 1-day workshop with an Approved Provider. Learners have 6 months from the completion on the online course to attend and complete the workshop. |

Resources A course Learner Guide provides guidance on relevant and current safety information in the resources sector. Additional videos, bulletins, and guidance notes are integrated as an extra educational aid to broaden knowledge in specific areas of interest. The ‘Ask the Panel’ Video Resource includes insights and experiences from safety professionals who manage and mitigate serious risks in their daily work roles or provide consultative guidance to companies to maintain statutory compliance. The forum topics align with the course modules and aims to provide an extension of information and solution-focused examples relevant to each topic. |
